What is a "Letter of Interest"?
Each physician will only be able to accept about 300 patients. To guarantee that you and your family's benefactor application will be considered, we request a completed "Letter of Interest" and a fully refundable deposit of $100 per person mailed to St. Luke's Family Practice, 1400 Florida Ave Suite 109, Modesto, CA 95350.

How much do I have to pay each year to be a benefactor?
Benefactors must pay the full cost for their medical service contract on or before December 31 to guarantee coverage the following calendar year. The contract fee is prorated for Benefactors joining our practice mid-year.

St. Luke's Family Practice is a federal tax-exempt non-profit public benefit corporation, federal tax ID number 38-3681072. We depend on our Benefactors generosity to support our primary mission: providing free outpatient healthcare, consistent with the Catholic tradition, to uninsured persons ineligible for government programs.

Why is it so expensive to be a benefactor?
Benefactors enjoy unparalleled access to their personal physician. Vastly smaller practice sizes for each doctor will allow same day service. There is no additional charge for any services Benefactors received from office. Most importantly, Benefactors will support the doctors of St. Luke's to provide no cost outpatient primary care for the uninsured of our community. Insurance

Won't I have to keep my medical insurance?
Absolutely. All Benefactors must maintain medical insurance. However, most Benefactors will be best served by high-deductible PPO (preferred provider organization) insurance like Blue Cross or Blue Shield. If you are currently purchasing your own HMO or PPO insurance, switching to a high-deductible plan may cut your premiums by half or more! Please contact Hibl Insurance Services.

I'm on Medicare (or Tricare) Can I become a benefactor?
Certainly. Because St. Luke's Family Practice will not charge for any services, you may continue to use your current Medicare (or Tricare) benefits for your other health expenses. However, St. Luke's Family Practice cannot participate in Medicare HMO programs like Secure Horizons or Kaiser Senior Advantage. Please contact Hibl Insurance Services with your questions.

When did St. Luke's Family Practice begin operation?
We began caring for benefactors January 1, 2004, and began providing free outpatient healthcare to uninsured recipients in April 2004.

 

Operations

Where is St. Luke's Family Practice located?
St. Luke's Family Practice is located at 1400 Florida Ave, Suite 109. Directly across the street from Doctor's Medical Center on the corner of Orangeburg and Florida Avenues.

What are the hours of operation?

  • Regular business days (except Wednesdays) from 8:30 am - 12:00 pm and 1:30 - 4:30 pm
  • Closed the week of Christmas and the week following Easter
We are available to benefactors 24 hours a day, seven days a week.
Wednesday, weekend, evening and holiday appointments by special arrangement.

How do I get a message to my doctor?
For less urgent messages we prefer to receive messages by email. Our benefactor website provides secure encrypted email that will be compliant with HIPAA regulations. Direct phone access to the doctor, voice mail messaging and fax is also available.
