We appreciate the support of our many benefactors and community supporters. Since our inception in 2004, we have conducted more than 6,250 uninsured office visits and provided over $500,000 in free care. Additionally, we have made thousands of referrals for care at other community health care agencies. Thank you for your generosity.

non-profit public benefit corporation under Section 501(c)(3)
of the IRS code. We are especially thankful for the support

Benefactor Letter 2007
St. Luke's Family Practice is open for service to benefactors
and uninsured persons.
- Business days (except Wednesdays) from 8:30 am - 12:00 pm and 1:30 - 4:30 pm
- Uninsured persons walk-in at or before 8:15 am or 1:15 pm for available appointment times
- Wednesdays, weekends, evenings and holidays for benefactors by special arrangement
St. Luke's Family Practice is located at 1400 Florida Ave, Suite 109, directly across the street from Doctor's Medical Center on the corner of Orangeburg and Florida Avenues. Our office phone number is 549-1057.
Benefactors can obtain usernames and passwords to access our enhanced website. You can schedule your own appointment, send a secure email to your doctor, change your account information and obtain important emergency contact information for your doctor 24 hours a day, seven days a week!
